JOB DESCRIPTION
1. Unit Name: Case Management – RN Case Manager
2. Brief Unit Description: Coverage for inpatient medical surgical.
3. Years of Experience Required: 10 years.
4. Skill Sets Required: Case Management, Utilization Management, and Discharge Planning.
5. Clinician to Patient Ratio: 1:15.
6. Patient Population: Adult.
7. Bed Count: 200.
8. EMR (Charting System): EPIC.
9. Floating Required: Yes. 1
0. Rotating Shifts Required: No
11. Certifications Required: Active MA RN License. Certified Case Manager preferred. BLS, ACLS.
12. Additional Information: Weekend commitment – up to every other weekend
